Help shape better integrated care

MULTIPULM’s Community of Practice is open!

People with lived, professional and policy experience in Brazil, Serbia and Türkiye are invited to help make MULTIPULM’s approach more relevant, practical and sustainable.
People with lived, professional and policy experience in Brazil, Serbia and Türkiye are invited to help make MULTIPULM’s approach more relevant, practical and sustainable.

Living with more than one long-term condition can mean repeated appointments, disconnected information and support that does not always fit together seamlessly.

For people living with a chronic respiratory disease alongside conditions such as diabetes, hypertension, heart disease or depression, fragmented care can make everyday life even harder.

MULTIPULM was created to address this challenge. The project is developing and implementing an integrated, digital care ecosystem to support people living with multiple long-term conditions, including at least one chronic respiratory disease, in Brazil, Serbia and Türkiye.

To make sure this work reflects real needs rather than assumptions, registration is now open for MULTIPULM’s Community of Practice.

Why Communities of Practice matter

Digital health tools are most useful when they are shaped alongside the people who will use, recommend, deliver and support them. A solution may appear effective on paper but still fail in practice if it does not fit local care pathways, available resources, everyday routines or the needs of different communities.

MULTIPULM’s Community of Practice will create an ongoing space for dialogue between people with lived experience, informal carers, healthcare professionals, researchers, technology experts, policymakers and health authorities.

By bringing these perspectives together, the Community of Practice will help the project better understand what coordinated care should look like in different settings and what is needed for useful approaches to continue beyond the project.

Who can take part?

The Community of Practice is open to adults with relevant lived or professional experience connected to healthcare in Brazil, Serbia or Türkiye.

  • People living with multiple long-term conditions: including at least one chronic respiratory disease, such as asthma or chronic obstructive pulmonary disease (COPD). Their lived experience can help the project understand where care feels fragmented and what would make daily management easier.

  • Informal carers and supporters: including family members, partners and others who provide unpaid practical or emotional support. They often see challenges that may not be visible during a clinical appointment.

  • Healthcare professionals: such as doctors, nurses, pharmacists, psychologists, physiotherapists and care coordinators, who understand what monitoring, follow-up and coordinated care require in practice.

  • Researchers, academics and technology specialists: who can contribute evidence, methods, critical reflection and expertise in the responsible development and implementation of digital health solutions.

  • Policymakers, health authorities and other relevant stakeholders: who can connect local experience with wider health-system priorities and help explore how successful approaches could be sustained or scaled.

How members can contribute

The Community of Practice is more than a mailing list. Depending on their interests, experience and the opportunities available in each country, members may be invited to:

  • share perspectives on the challenges of managing chronic respiratory disease alongside other long-term conditions;

  • take part in webinars, discussions, surveys, focus groups or local workshops;

  • provide feedback that can inform the design, communication and implementation of MULTIPULM’s digital tools and integrated care approach;

  • exchange knowledge with people from different professional and personal backgrounds;

  • help ensure that future guidance and policy recommendations reflect local realities; and

  • support peer learning, training or local implementation through more active roles later in the project.

*Please note: Joining a Community of Practice does not automatically confirm participation in every workshop, research activity or tool-testing opportunity. Some activities may have specific eligibility criteria or limited places, and additional information or consent may be required.

Why take part?

Members will have a chance to bring real-life knowledge into a major international research and innovation project. Their perspectives can help MULTIPULM identify barriers early on, communicate more clearly and develop approaches that are better suited to the communities and health systems where they will be used.

Participation also offers opportunities to learn from others, connect across sectors and contribute to a wider conversation about more person-centred, coordinated and digitally supported care.

For MULTIPULM, this involvement is essential. The project’s long-term value will depend not only on the technology it develops, but also on whether people trust it, can use it and see a realistic place for it within everyday care.

How to join

Registration takes only a few minutes. Visit the MULTIPULM Community of Practice page and select the registration form for the country most relevant to you.

After registration, the relevant community team will contact members with information about suitable activities and opportunities to participate.

Looking ahead

In the coming months, the Community of Practice will grow through local engagement activities, workshops, focus groups, webinars and opportunities to share feedback with the MULTIPULM team.

This is the beginning of a network built around people’s experience of care and the knowledge of those working to improve it. By connecting lived experience, clinical practice, research and policy, MULTIPULM aims to support integrated care that is not only innovative, but also useful, inclusive and realistic.
