The consortium develops integrated, sustainable care solutions for people living with chronic respiratory conditions and multimorbidity.

The project is powered by a cross-sectoral alliance of SMEs, universities, research institutes, healthcare providers, NGOs,
and public authorities. This collaborative structure ensures that digital innovation, scientific research, implementation expertise, and policy alignment are embedded at every stage.
Pilot programmes in Brazil, Serbia, and Türkiye are co-developed and led by national partners, ensuring each solution reflects local healthcare realities and can be adopted sustainably in low- and middle-income settings.
