M-CARE is an initiative focused on improving healthcare for people with multiple chronic conditions and mental health challenges in Sub-Saharan Africa.
 

This project works across Ghana, Kenya, and Uganda, adapting WHO’s evidence-based guidelines to create integrated, patient-centered care at the primary health level.
 

Their mission is to strengthen early detection, treatment, and long-term care, helping reduce premature deaths from non-communicable diseases and building sustainable health systems across Africa.

HIVE is a Horizon Europe Research and Innovation Action that aims to improve the management of HIV and non-communicable diseases (NCDs) through smart, inclusive, and sustainable healthcare.

By combining digital tools, interpersonal counselling, and implementation research, HIVE brings together experts, healthcare providers, and communities to strengthen health systems and enhance the wellbeing of people living with HIV (PLWH).

PEN-CONNECT is an EU-funded project working in Mozambique to strengthen health systems and improve how primary and district care support people living with multiple long-term conditions.

EMPOWER is a Horizon Europe project (2025–2029) working in rural Benin and Togo to close critical treatment gaps for non-communicable diseases (NCDs) by strengthening frontline care where it is needed most.

It equips community health workers and primary healthcare providers with evidence-based training and an Android mobile tool to support community awareness, screening, education, referral, and long-term follow-up, for people living with multiple long-term conditions.
